Medication

Prescription Medication:

All prescription medications require a doctor's order and signed parent permission. A form to be completed is available in each building's health office. The medication must be brought to the school in its original container with a label prepared by a pharmacist.

Over-the-counter Medication:

A medication permission form must be completed and signed by the student's parent for any OTC medication to be given at school. A note from the parent will be accepted if it contains the following information:

  • name of medication
  • dose (ie, how many milligrams, teaspoons or tablets)
  • time to be given
  • reason for medication
  • parent's signature

A doctor's signature is not required for OTC medicines. The medication must be brought to school in its original, sealed container. Medication cannot be admnistered if it is past the expiration date.

Inhalers for Asthma:

Minnesota State Law now permits a student to carry an inhaler if he/she meets specific criteria. Please discuss this with the nurse at your child's building. Rules for prescription medication (above) still apply.
